If you wish to hold your wedding at First Parish Brewster, you need to fill out two forms and send them to First Parish.

The forms are:

• Information About You

  and

• The Services You Would Like First Parish To Provide

To print out each form, click the links to the forms above and press the "Print" button on each form. Your printer should print out each form.

Then, fill out both forms and send the completed forms to:
